Who is on the care team and how to reach them
For caregivers and staff in this community.
This page shows who does what on a typical care team and who to reach during a shift. It does not replace your community's care plans or escalation procedures. If a resident's condition changes or you are unsure, tell the charge nurse right away, then document what you observed the way your community requires.
Owner: the community director. Review: whenever a team member joins, leaves or changes role.
