Overview

Benefits & Total Rewards is a 12-page intranet site template that helps employees find the right benefits information and understand where decisions or actions belong. The Benefits home page introduces the hub and links to plan comparisons, enrollment preparation, help, retirement savings, perks, EAP support, life events, new-hire guidance, hourly and part-time benefits, total rewards, and a contacts directory. The site navigation groups these pages around employee tasks and common situations.

The pages use a fictional Northstar Services example to show how to organize practical guidance. Employees are prompted to compare materials for the same plan year and audience, record unanswered questions, check current instructions, and use the designated enrollment system for elections. Other pages distinguish employer-owned questions—such as eligibility or enrollment timing—from carrier questions about claims, providers, or ID cards. Life-event guidance also separates benefits, leave, and payroll questions so readers do not assume one service determines another.

Use this template when you need a structured orientation site that points employees to approved sources and support routes. Before launch, replace the fictional examples with verified organization-specific information and confirm app access, ownership, correction routes, and review triggers. Do not use the sample figures, plan descriptions, dates, contacts, or eligibility examples as actual terms. This site is not a coverage decision tool, legal advice, enrollment transaction, or replacement for official plan documents; avoid using it when those authoritative materials are unavailable or unverified.

Standards & compliance context

  • Treat this template as an orientation layer and retain official plan documents and approved enrollment instructions as the sources for plan terms and decisions.
  • Have qualified reviewers confirm that eligibility, enrollment deadlines, life-event rules, and location-specific wording match the applicable approved materials before publication.
  • Check that any personal or health information is collected only through approved systems and is not requested through public pages, comments, or group channels.
  • Review the site for WCAG 2.1 AA accessibility, including keyboard navigation, headings, link descriptions, image alternatives, and readable contrast.
  • Verify local emergency and crisis resources for each intended audience and location before publishing; do not present an unverified sample resource as applicable guidance.

General regulatory context for orientation only — verify current requirements with counsel or the relevant agency before relying on this template for compliance.

Start with the hub’s task links, then verify that any plan or offer information belongs to the current approved source.

  • Make your benefits choices with clarity
Northstar Services employees can use this hub to compare approved plan materials, prepare for enrollment, and find the right support route. Northstar Services is a fictional example. Use current approved organization materials for plan-specific answers and decisions.
  • Benefits at a glance
Example figures for a fictional plan year — replace them with yours. Medical plans 3 Preventive care covered 100% Retirement match 4% Confidential EAP line 24/7
  • Choose your next step
Go directly to a benefits task or support reference. Compare medical plans Review the same plan-year details across official materials. Prepare for enrollment Organize your questions and follow the current enrollment instructions. Understand terms and get help Check common vocabulary and prepare a focused question. Life Events & Benefit Changes Qualifying life events, the change window, what to submit. New Hire Benefits Guide Your enrollment window, what to have ready, where to enroll. • Perks and offers are separate from plan coverage
If Northstar Services provides an optional offer, check its current approved source before relying on it. Check who the offer applies to Look for the eligible audience, locations, or other stated conditions in the offer source. Ask the benefits owner when applicability is unclear. Identify the accountable owner Use the owner named in the current offer information for questions about terms or administration. Verify the expiry Check the stated end date or renewal terms in the current source; do not rely on an old announcement or saved copy.
  • Check that the documents belong together
Worked example: An employee finds a medical plan summary labeled “Plan Year 2025” and an enrollment overview labeled “Plan Year 2026.” Rather than comparing them as a matched set, the employee notes both titles and asks the benefits team which current approved materials apply to their situation. The team’s confirmed source set—not the employee’s guess—becomes the basis for the comparison. This hub organizes questions; it does not determine coverage, eligibility, or which option is right for an individual.

Compare medical plans

  • Compare the same plan year and applicable audience
Start with current approved materials for the same plan year and the audience they describe. Use the source wording for plan-specific details. If a source does not answer a question, record “not stated” and ask the benefits team rather than filling the gap with an assumption.
  • Questions to verify in official plan materials
This is a question reference, not a plan comparison or statement of coverage. Question Where to verify What to ask if unclear Do both materials identify the same plan year and applicable audience? Document title, plan-year label, and stated scope Which approved document set applies to my situation? How are employee costs described, and what payment frequency is stated? Current official plan materials and enrollment instructions Which cost figures and payment schedule should I use for this plan year? Where does the source explain provider or network information? The plan material’s provider or network reference Which official source explains how to verify provider information? What coverage limits, exclusions, or conditions does the source state? Relevant coverage description, definitions, and exclusions in the official material Which section defines this term or condition for this option?
  • What each plan is best for
Core Lowest paycheck cost Higher deductible; pairs with a health savings account. Standard Balanced Moderate deductible and copays for regular care. Plus Most predictable costs Lowest deductible; highest paycheck contribution.
  • A matched comparison starts with matched sources
These fictional document labels illustrate a process check; they do not represent available Northstar Services plans. Useful comparison setup Two approved summaries both identify Plan Year 2026 and the same stated audience. The employee records the document titles and checks the same question in each. Confirm the source versions are current. Compare like-for-like sections. Record unanswered questions with the section checked. Pause before comparing One summary says Plan Year 2025 and another says Plan Year 2026. The employee asks the benefits team to identify the applicable approved set before drawing conclusions. Do not combine mismatched plan years. Do not infer which source applies. Keep the question tied to the document titles.
  • Continue with preparation or definitions
Prepare for enrollment Use the enrollment sequence to organize next steps. Understand terms and get help Review vocabulary and how to phrase an unresolved question.

Enrollment journey

  • Start with the applicable enrollment instructions
Before gathering details or entering choices, use the current approved instructions to identify the plan year, audience, dates, and designated enrollment system that apply. The sequence below helps you prepare; it does not determine eligibility or submit an election.
  • Prepare for enrollment
Use the organization’s approved instructions for dates, required information, and submission. Find the applicable instructions Use the current approved enrollment instructions to identify the audience, plan year, dates, and designated enrollment system. Gather matching plan materials Check that the materials refer to the same plan year and the audience described in the instructions. Write down your questions Note the document title and section checked, then state what remains unclear. Use Compare medical plans for a consistent set of questions. Use the designated enrollment system Follow the current instructions in the organization’s actual enrollment system. This page does not collect or submit elections. Keep the system’s confirmation Retain the confirmation or record provided by the enrollment process, following the organization’s instructions for where to keep it.
  • Life-event questions may cross service owners
A change in circumstances can raise separate questions. Each owner answers within their service; no single handoff establishes another service’s rules. Benefits owner Ask which approved benefits and election guidance applies to the event and where its current source is maintained. Leave owner Ask the leave team about time-away processes, scheduling questions, and the authoritative leave guidance. Payroll owner Ask payroll how to find the applicable guidance for payroll-record questions. Do not infer a payroll change from benefits or leave information.
  • Worked example: preparing for parental leave
An employee preparing for parental leave has three distinct questions. Each question includes only the context needed to route it; none assumes eligibility or a particular benefit, leave entitlement, or payroll outcome. The employee uses each owner’s current approved source for that service. These are separate handoffs, not an automatic cross-service workflow. Which approved election instructions apply to my situation? The employee sends the first question to the benefits owner. Benefits owner Where do I find the process for discussing time away? The employee sends the time-away process question to the leave owner. Leave owner Which team can explain my payroll-record question? The employee sends the payroll-record question to the payroll owner. Payroll owner

Benefits terms and help

  • When sources disagree, ask which one applies
Northstar Services is a fictional example. For a benefits question, note the source titles and plan-year labels, then ask the benefits team which current approved source applies to your situation. For an enrollment-system access issue, use the technical-support route named in the current enrollment instructions.
  • A short guide to benefits source language
Use the applicable approved source for decisions; these general explanations do not define plan terms. Term Reading aid Where the applicable meaning comes from Plan year The period identified by a plan’s official materials. The current approved plan materials and enrollment instructions. Official plan document An organization-approved source describing plan terms and conditions. The current source identified by the benefits owner for the employee’s situation. Enrollment confirmation A record provided by the enrollment process after the employee follows its instructions. The designated enrollment system and current organization instructions. Eligibility Whether a person or situation meets applicable conditions; this hub does not decide it. The relevant approved source and the responsible benefits or service owner.
  • When the next step is not obvious
What should I do when two benefit documents disagree? Note both document titles and any plan-year labels, then ask the benefits team which approved source applies. Do not choose one based only on its date or infer which terms govern. What if I cannot access the enrollment system? Use the technical-support route named in the current enrollment instructions. Include the step where access failed and any non-sensitive error wording; never send a password or other credential. Can this hub tell me whether I am eligible? No. Check the applicable approved source and ask the responsible benefits or service owner to clarify how it applies to your situation.
  • A focused benefits question
Fictional example message “I found a plan summary labeled Plan Year 2025 and an enrollment overview labeled Plan Year 2026. Which approved source set applies to my situation? I have not used these documents as a matched comparison. Please point me to the current source to check.” Who to ask about sources The benefits team owns guidance on which benefits source to consult. If this page or its wording is unclear, raise the correction with the benefits team. Enrollment-system access issues For an enrollment-system access issue, use the technical-support route in the current enrollment instructions.
  • Continue to a benefits task

Our Total Rewards Philosophy

  • How our total rewards fit together
A Northstar example for office and frontline employees Fictional example: Northstar is a multi-location employer. Total rewards brings together pay, benefits, and recognition—the different ways the company compensates work, supports employees, and acknowledges contributions. The mix is not identical for every person. Role, location, eligibility, and the terms of an applicable plan or program can affect what is available. This page explains the general idea; it does not replace approved pay guidance or benefit plan documents.
  • Three parts of the rewards experience
These examples describe distinct parts of total rewards, not a promise that every program applies to every employee. Pay Pay is compensation for work. In this fictional example, role scope and level help provide a consistent starting point for considering pay. Check the compensation information available to you in Compensation Management. Use approved company guidance for questions about your pay. Benefits Benefits can support employees in different ways, but available options and eligibility depend on applicable plan terms and location. Check the Benefits & Total Rewards site for current benefit information. Read the applicable plan details before making an enrollment decision. Recognition Recognition can acknowledge a specific contribution or milestone. Programs and participation may differ by location or role. Name the contribution and its effect when recognizing a colleague. Check company guidance for available recognition programs.
  • How a pay decision might be considered
Illustrative Northstar example only—not a statement of approved Northstar policy. Confirm Northstar's actual factors, reviewers, and communication steps before publishing this as company practice. Define the role Describe the work, responsibilities, and level so the decision starts with the role rather than a job title alone. Review relevant context In this example, reviewers consider relevant market information and internal consistency. The information used and how it is weighed require company confirmation. Consider applicable factors Performance and budget may be considered where applicable. Their relevance, timing, and effect must be confirmed against Northstar's actual process. Communicate the outcome Explain the decision through Northstar's approved communication process and direct employees to Compensation Management for information or actions available to them.
  • Check Compensation Management
Use the Compensation Management app for compensation information and related actions available to you. No app link is supplied in this example. Connect the app through the existing site setup and confirm employee access and available features before publishing. Open Compensation Management from the apps available to you. The information and actions shown depend on Northstar's configuration and your access. If you cannot find the relevant information, use Northstar's approved help route.
  • Check what applies to you
Do not assume that an example on this page establishes your eligibility or a guaranteed payment or benefit. What can vary Pay practices, benefit options, eligibility, and recognition programs may vary by role, location, and applicable plan or program terms. Where to look For benefit details, go to the Benefits & Enrollment Hub and review the current guidance and plan information that applies to you. For pay information, use Compensation Management. If either source does not answer your question, follow Northstar's approved help route. Publishing checks These details are not supplied in this fictional example. Confirm the actual pay decision process and its communication route. Verify applicable benefit guidance and recognition program details. Identify the content owner and correction route. Set a review trigger, such as a change to approved compensation or benefit guidance.

Retirement Savings

  • Understand the plan before you enroll
A practical starting point for Northstar employees Fictional example: Northstar is a sample multi-location employer, and the plan terms and figures on this page are illustrative only. Eligible employees in this example can save through payroll and may receive an employer match. Check Benefits Management for the current plan documents, eligibility rules, available options, and enrollment instructions that apply to your employee group and location. Those official materials—not this example page—control your choices.
  • How the sample match and vesting work
These Northstar figures are examples, not actual plan terms. Illustrative employer match For this example only, Northstar matches 100% of an employee's contributions on the first 3% of eligible pay, then 50% on the next 2%. An employee contributing 5% would receive a sample employer match equal to 4% of eligible pay. The example assumes the employee meets the plan's eligibility requirements. The actual match formula, eligible compensation, limits, and timing must be confirmed in the current plan documents in Benefits Management. Illustrative vesting schedule For this example only, Northstar's employer match becomes vested over three years: 33% after one year of service, 67% after two years, and 100% after three years. Employee contributions are shown as immediately vested in this example; confirm how the actual plan treats each contribution type. The plan document defines service credit, vesting dates, and any exceptions.
  • Enroll through Benefits Management
This page explains where to act; it does not submit or change an election. Open Benefits Management Sign in to Benefits Management using your organization's normal access route. If the app is unavailable, use your usual internal support channel. Check eligibility and plan materials Review the eligibility information and current official plan documents shown for your employee group. Ask Northstar Benefits if your location or employment type is not clearly covered. Review your elections Compare the available options, contribution limits, investment choices, and any applicable deadlines in the app before deciding. Submit and verify Enter your contribution and investment elections in Benefits Management, submit them there, and check the app for its confirmation or next steps.
  • Get help with plan questions
  • Use the official plan documents
Know where to look and who to ask Read the plan documents first Before making or changing an election, read the current plan documents in Benefits Management. They provide the authoritative details on eligibility, match calculations, vesting, limits, and any location- or employee-group-specific terms. Investment, balance and transaction questions For questions about investment options, account balances, or transactions, use the provider support route listed in the approved plan materials. Eligibility and finding materials For questions about Northstar eligibility or how to find the right materials, contact the Northstar Benefits team through your organization's established support channel. Confirm and add approved contact details to this page before publishing it as a support directory.

Perks & Employee Discounts

  • Find current offers in Company Store
For Northstar office and frontline employees Northstar Services is a fictional example. Company Store is the place to browse available employee discounts and partner offers and read the instructions for using them. Offers may vary by location, eligibility, and availability. Before acting on an offer, check its current terms, eligible audience, any location limits, and the redemption instructions shown in Company Store. This page is a guide; it does not process redemptions.
  • Example offer categories
Illustrative categories only—not confirmation of current Northstar offers. Apparel A fictional example category for workwear or everyday clothing offers. Check Company Store for any current offer and its eligibility and terms. Confirm which items are included. Check the stated expiry and redemption instructions. Travel A fictional example category for travel-related partner offers. Availability and restrictions may differ by offer and location. Review travel dates and booking conditions. Check whether the offer applies to your intended location. Local services A fictional example category for services near participating Northstar locations. Do not assume a local partner participates. Look for the covered service area. Follow the offer-specific instructions in Company Store.
  • How to use an offer
Check the offer details before making a purchase or booking. Open Company Store Use the Company Store app available through your organization’s MangoApps setup. If you cannot find or open it, use your established internal support route. Browse available offers Search or browse the categories shown in Company Store. The offers visible there may depend on the current program and its stated eligibility. Read the full terms Check eligible employees, locations, included items or services, expiry, restrictions, and any required steps before purchase. Follow the offer instructions Use the redemption method described in Company Store. This page does not provide a separate redemption process.
  • Browse current discounts
Company Store is the destination for current offer details and redemption instructions. No offer is guaranteed to be available to every employee or at every Northstar location. Open Company Store from your MangoApps setup and review the offer details there.
  • Common questions
Use the offer listing in Company Store as the source for current terms. Are the categories on this page current offers? No. Apparel, travel, and local services are fictional examples to illustrate possible categories. Check Company Store for offers actually available to you. Can every employee use every offer? Do not assume so. Eligibility, location coverage, and availability can vary. Read the specific offer terms in Company Store before relying on it. Where do I find redemption instructions or restrictions? Review the offer listing in Company Store. Follow the instructions and check all restrictions stated there before making a purchase or booking. What if I cannot access Company Store or need help? Use Northstar’s established internal support route for access help or questions. The organization adding this page should identify the correct help route and content owner before publication.

EAP & Mental Health Support

  • Confidential support starts with the EAP
For Northstar office and frontline employees Fictional Northstar example. The Employee Assistance Program (EAP) is a place to ask for support with personal, family, or work-related concerns. To contact the EAP, open the existing Benefits & Enrollment Hub and find the current provider contact and access instructions. The EAP provider—not MangoApps—handles counseling and related services. Before sharing personal information, ask the provider how confidentiality works, what exceptions may apply, who is eligible, and what services are available under the current plan. Do not assume that an employer can or cannot see particular information without checking the provider's stated terms.
  • Support the EAP may offer
Illustrative Northstar plan example only; confirm actual services and eligibility on the Benefits & Total Rewards site. Short-term counseling The fictional example plan includes access to short-term counseling for concerns such as stress, anxiety, or changes at home or work. Ask the provider about the number or type of sessions included. Confirm whether remote and in-person options are available in your location. Example Family, grief, and caregiving concerns The example includes support for relationship difficulties, bereavement, and caregiving questions. Ask whether eligible family members can contact the service. Check whether referrals or additional services have separate terms or costs. Referrals and next steps An EAP may help identify other resources when a concern needs support beyond the services it provides. Ask what happens if ongoing or specialized care is recommended. Confirm any referral, eligibility, and payment details directly with the provider.
  • How to reach the EAP
The provider's current instructions on the Benefits & Total Rewards site are authoritative. Open the Benefits & Total Rewards site Find the EAP entry and read the current contact and access instructions. Do not rely on an old saved number or an example from this page. Contact the provider directly Use the contact method shown in the hub. The provider will explain how to begin and what information is needed. Ask what applies to you Confirm confidentiality terms and exceptions, eligibility, available services, language options, hours, and any costs or limits before proceeding.
  • If you need urgent help
Immediate danger If you or someone else is in immediate danger, call your local emergency services number now or go to the nearest emergency department. Do not wait for an EAP appointment or a response through MangoApps. Verify crisis resources before publishing Location-specific crisis resources must be verified before publishing. The U.S. 988 Suicide & Crisis Lifeline is an example for people in the United States; confirm the current number, availability, languages, and suitability for the employee's location before relying on or publishing it. If you are unsure which resource applies Employees elsewhere should use a verified crisis service for their location. If you are unsure which resource applies, use local emergency services for immediate danger and check the organization's confirmed location-specific guidance.
  • Next step
Find the current provider contact and plan terms on the Benefits & Total Rewards site. This page is a guide, not a replacement for the current plan details. Benefits content owner: confirm the responsible role and help route before publishing. Review when the provider, coverage, eligibility, confidentiality terms, or location-specific crisis resources change. Open the Benefits & Total Rewards site Find EAP contact and access instructions Ask the provider what applies to you

Life Events & Benefit Changes

  • Life events may give you a chance to change benefits
For Northstar office and frontline employees Start in Benefits Management. Review the event and the submission deadline in your current Northstar plan information, gather the details and documents requested, then submit your change in Benefits Management. This page is an orientation guide; it does not confirm eligibility or submit a request. Illustrative example only: Northstar's sample plan uses a 30-calendar-day window after an event. This is not a confirmed Northstar rule. Before acting, check the current official plan terms for the applicable window, how the event date is counted, and any location-specific rules. The Benefits & Enrollment Hub is the starting point for Northstar's official benefits information. Use its current plan materials to confirm the rules that apply to you. If the instructions in Benefits Management or the official materials are unclear, contact the benefits support route identified in those materials.
  • Events that may allow a benefits change
These are common examples, not a guarantee that a particular change is available under your plan. Marriage A marriage may allow a change to certain coverage or dependent information. Check the current plan rules for who may be added and what proof is accepted. Note the event date. Check whether the plan allows the change you need. Birth or adoption A new child may qualify for a coverage change. Confirm the applicable deadline and the documents requested for the specific event. Record the birth or placement date. Review dependent details before submitting. Change in a spouse's or partner's coverage A spouse's or partner's coverage change may affect your available options. Eligibility and accepted documents depend on Northstar's plan terms. Note when the other coverage changes. Check which proof of the change is required. Loss of other coverage Losing other coverage may allow a benefits change, subject to the plan's rules and submission window. Record the date coverage ends. Gather the notice or other evidence requested by the plan.
  • Prepare and submit your request
Use this sequence, then follow the current instructions in Benefits Management. Record the event date Write down the date the event occurred or the other coverage changed. The plan terms determine which date starts the clock. Check your submission window Open the current official Northstar plan information and confirm the deadline, whether it is calendar or working days, and how the event date is counted. Gather requested information Prepare the affected dependent's details and any event or coverage evidence named in the plan instructions. Do not send sensitive documents through an unapproved channel. Submit in Benefits Management Open Benefits Management, choose the life-event or benefit-change task shown there, and follow its prompts. Review the request and any confirmation or follow-up instructions.
  • What to have ready
Requirements vary by event and plan. Treat this as a preparation checklist, then use the current plan instructions and Benefits Management prompts as the authority. Event and coverage details Have the facts needed to describe what changed. Event type and event date. Names and relationship of dependents affected. For other coverage changes, the coverage end or change date and the affected person. Supporting documents Submit only documents the current instructions request. Check the accepted document types for your event. Use the upload method provided in Benefits Management. If a document is not yet available, check the plan instructions or ask the designated benefits support route what to do before the deadline.
  • Start or review a change
Benefits Management is the app used to submit a life-event benefits change. This page does not verify your eligibility, change coverage, or replace the official plan terms. Open Benefits Management from your available company apps. Select the life-event or benefit-change task presented in the app. Follow the app's prompts and retain its confirmation or follow-up instructions.
  • Questions employees often ask
For a decision about your coverage, use the current Northstar plan information and Benefits Management instructions. When does changed coverage take effect? The effective date depends on the event and Northstar's plan rules. Check the applicable plan information and the confirmation in Benefits Management; do not assume coverage starts on the event date or the submission date. What if I do not have a required document yet? Check the current instructions for the event and contact the benefits support route identified in the official materials before the submission window closes. Do not assume a request is complete or that a later document will be accepted unless the official instructions confirm it. How do I know the deadline that applies to me? Confirm the submission window, day-counting method, and event-date rule in the current Northstar plan information. The 30-calendar-day example on this page is illustrative only and is not a policy deadline. Where do I confirm the official rules? Use the current benefits materials in Northstar's Benefits & Total Rewards site and the instructions shown in Benefits Management. If the rules are unclear or appear to differ by location, use the help route named in those materials before submitting.

New Hire Benefits Guide

  • Check your enrollment window first
A practical guide for new Northstar employees Northstar is a fictional example company. Enrollment dates and details on this page are illustrative, not actual Northstar policy. Your personal deadline, eligibility, and available plans may depend on your role, location, and the terms shown for you. Use the deadline shown there rather than relying on an example date or a general onboarding schedule.

The example dates below are fictional and do not set a real enrollment period. Check your personal window In Benefits Management, review the enrollment deadline, eligibility details, and any instructions that apply to your employee group or location. Gather the information requested Use the app's current checklist. Depending on the plan and your situation, you may need dependent details or information about other coverage. Confirm submission status Return to Benefits Management and check that your elections show as submitted. Follow any additional instructions displayed there.

Your personal details Have the information the enrollment checklist requests so you can complete the relevant fields accurately. Check the app for required fields before you begin. Dependent details, if applicable If you plan to enroll eligible dependents, gather the details requested by the app. Check its instructions for eligibility and required documentation. Follow the prompts shown for your plans rather than assuming this applies to everyone. Use the current app checklist to confirm what is needed.
  • Where to enroll
Complete benefits elections in Benefits Management. If you cannot access the app or cannot find the instructions for your situation, use the approved benefits help route provided by your employer. • Continue onboarding in Onboarding Hub
Completing an onboarding task is not a substitute for submitting elections in Benefits Management. App Use it for Benefits Management Completing your benefits elections Onboarding Hub Your other onboarding tasks and guidance
  • Example timing and publishing notes
Illustrative timing only: A fictional Northstar employee might see a 30-day enrollment window in Benefits Management. That example is not a deadline for any real employee. Check the window shown for you in the app. Before publishing: The benefits team should confirm current enrollment windows, eligibility rules, required information, plan options, and any differences by location or employee group. Confirm that office and frontline employees can access both apps and add the approved help and correction route. Add this page to the existing Benefits & Enrollment Hub and configure app connections or destinations through the appropriate setup process.

Benefits for Hourly & Part-Time Employees

  • Check your benefits details in Benefits Management
For Northstar hourly and part-time employees Eligibility can depend on hours worked and the benefit. Your scheduled hours alone may not answer whether you qualify; the applicable rules and measurement period matter too. Check Benefits Management for the information that applies to you. Northstar is a fictional company, and this page is an illustrative example—not a statement of actual eligibility rules. Do not use sample details or another employee’s situation to determine your coverage.
  • What to review
These details can help you understand what Benefits Management is showing. The rules vary by benefit and must be confirmed in your employer’s current plan information. Hours and schedule Compare the hours recorded for you with your current schedule. A schedule may not be the same as hours counted under a particular benefit’s rules. Check the date range shown in Benefits Management. If recorded hours look wrong, follow the app’s help instructions. Measurement period Some eligibility decisions use hours over a defined period rather than one week. Use the period and explanation shown for the benefit you are reviewing. Note the start and end dates displayed. Ask Benefits Management support if the period is unclear. Benefit and enrollment details Eligibility and enrollment timing can differ across benefit types. Check the specific benefit’s terms and the dates shown for your enrollment opportunity. Review the benefits available to you. Follow the displayed enrollment instructions and deadlines.
  • Find your personal information
Use Benefits Management for your current eligibility and enrollment details. Open Benefits Management Sign in using your organization’s approved access route and open the Benefits Management app. Review your eligibility details Choose the benefit you want to check. Read the hours information, measurement period, and eligibility explanation shown for you. Check enrollment dates and instructions Review any enrollment window and the actions available to you. Follow the app’s instructions before the displayed deadline. Use the app’s help route if something is unclear For a question about your personal record, eligibility result, or next step, use the support route shown in Benefits Management. Include the benefit and date range you reviewed.
  • Your next step
Benefits Management is the source for your personal eligibility and enrollment information. Northstar sample setup note: before publishing, confirm the eligibility rules, measurement periods, benefit types, enrollment windows, and any location- or role-specific differences. • Who to call for your benefits question
For Northstar office and frontline employees at every location Call the carrier for questions about a specific claim, an ID card, a provider search or a pharmacy. Contact the Northstar Benefits Team for questions about whether you are eligible, when you can enroll or change coverage, and what comes out of your paycheck. Before you call, check the current plan documents in the Benefits & Enrollment Hub. Plans and contacts can differ by employee group and location, so use the row that matches your plan. Every carrier, plan name, phone number and group number on this page is a fictional example for Northstar. The Benefits Team owns this page and updates it when a carrier, plan or phone number changes. If a number here does not work for you, tell the Benefits Team so it can be corrected.

  • Which contact fits your situation
Example routing for Northstar. Replace with your own contacts and turnaround times. A claim is pending, denied or looks wrong Call the carrier listed for that coverage. The Benefits Team cannot see or change claim decisions, but it can help if you are not getting an answer. Medical, dental and vision claims go to the carrier's member services line. Ask for a reference number for the call and write it down. If a denial is unresolved after 15 business days, send the Benefits Team the claim number and date of service. You need an in-network doctor, dentist or pharmacy Use the carrier's provider search on the member site or call member services. Confirm the provider is in network for your specific plan name before you book. Say the plan name from the table, such as Northstar PPO 500. Ask whether the provider is in network at the exact address you plan to visit. You need an ID card or your member ID Request a replacement through the carrier's member site or by phone. Digital cards from the carrier's app are usually accepted at appointments. New hires: cards typically arrive 10 to 14 days after enrollment is processed. If your card has not arrived by then, call the carrier first, then the Benefits Team. You are not sure you are eligible, or need to change coverage Contact the Northstar Benefits Team. Eligibility, enrollment windows, life events such as a new baby or marriage, and payroll deductions are decided by Northstar, not by the carrier. Frontline employees with variable hours should ask the Benefits Team how hours affect eligibility. Report a life event within 30 days (example period; confirm in your current plan documents).
  • Have these ready before you call
A few details usually shorten the call and avoid a second one. Find your plan name and member ID Both are on your ID card or in the carrier's member site. Match your plan name to the table above so you call the right number. Note the group or plan number Use the group number from the table when a carrier asks for one. Your member ID is different and is personal to you. Gather claim details For a claim question, have the date of service, the provider's name and the amount billed, plus the claim number if you have one. Write a one-sentence summary of the issue For example: "My 3/12 dental cleaning was billed as out of network, but my dentist is listed as in network." Keep personal details off public channels Do not post member IDs, diagnoses or claim details in group chats, team feeds or comments. Share them only with the carrier or the Benefits Team directly.

  • Before you call, check the Benefits & Total Rewards site
Current plan documents and enrollment information are kept there. Still unsure who to call? Ask the Northstar Benefits Team and they will point you to the right contact. How to use this template

  1. Clone the site and confirm the intended employee audiences, locations, benefits owner, correction route, and authoritative source for each topic.
  2. Replace Northstar and fictional app references with your confirmed organization names, systems, plan materials, support routes, and eligibility qualifications.
  3. Review every sample figure, plan description, deadline, carrier detail, crisis resource, and contact entry with the owner responsible for that information, removing anything you cannot verify.
  4. Confirm each internal destination and app connection through your site setup, then check that intended office, frontline, hourly, and new-hire audiences can access the relevant pages and systems.
  5. Publish the navigation and pages with a named content owner and a review trigger tied to changes in plans, dates, providers, eligibility, app behavior, or support guidance.
  6. Ask a representative employee to follow common tasks from the home page, record dead ends or unclear terms, and correct the pages before broad rollout.

Best practices

  • Keep current plan documents as the authoritative source and use the hub to orient employees to the right materials and owner.
  • Show plan year, audience, and source title together wherever employees are asked to compare benefit information.
  • Replace illustrative deadlines and plan figures with confirmed terms, or remove them rather than leaving examples that could be mistaken for policy.
  • Keep benefits, leave, payroll, carrier, and technical-support routes distinct, and name the owner for each type of question.
  • Verify every app and page destination in the published site; do not leave a callout that implies a working connection when none is configured.
  • Review directory entries, eligibility wording, offer expiry details, and location-specific crisis guidance whenever their approved source changes.
  • Use short task labels and keep the Benefits home quick links focused on employee actions rather than internal department names.
  • Check heading order, keyboard access, link purpose, image alternative text, and contrast against WCAG 2.1 AA before publishing.

What this template typically catches

Issues teams running this template most often surface in practice:

Employees compare documents with different plan years or audiences and draw conclusions from a mismatched set.
Illustrative deadlines, match figures, coverage descriptions, or sample phone numbers remain in place and look like current policy.
A page names an app or support route that is unavailable to the employee group trying to use it.
Carrier claim questions are sent to the benefits owner, or eligibility and enrollment questions are sent to a carrier that cannot decide them.
A life event is treated as one combined process even though benefits, leave, and payroll have separate owners and rules.
The contacts directory has no confirmed owner or correction route, allowing outdated carrier details to remain visible.
Employees share member IDs, diagnoses, or claim details in public channels instead of an approved private support route.
Long explanations bury the immediate next step or leave no clear route when a source does not answer the question.

Common use cases

HR benefits owner at a multi-location employer
Use the 12-page navigation to separate plan references, employee situations, wellbeing support, and contact routing. Replace the fictional examples with approved location- and audience-specific sources, then assign review ownership to the relevant service owners.
New employee preparing for enrollment
The new-hire page directs employees to check their own deadline, gather only the information requested, submit elections in the designated system, and verify submission status. It distinguishes onboarding tasks from benefits elections.
Hourly employee checking benefit eligibility
The hourly and part-time page prompts employees to review the benefit, hours period, measurement dates, and app-provided explanation. It avoids treating a scheduled weekly total or another employee’s case as an eligibility decision.
Employee experiencing a qualifying life event
The life-events page provides a preparation sequence: record the event date, verify the applicable deadline, gather requested information, and use the benefits system’s instructions. Its examples are explicitly nonbinding and direct employees to current plan rules.
Employee trying to resolve a claim or coverage question
The contacts directory separates carrier-owned claim, provider, and ID-card questions from employer-owned eligibility, enrollment, and payroll questions. Replace the fictional directory entries and sample routing details with verified contacts before employees rely on them.

Frequently asked questions

What does the Benefits & Total Rewards template include?

It includes a home page plus 11 topic pages covering plan comparisons, enrollment, terms and help, total rewards, retirement savings, perks, EAP support, life events, new hires, hourly and part-time employees, and benefits contacts. The pages use a fictional Northstar Services example and clearly distinguish illustrative content from approved plan information. Navigation groups the pages by plans, wellbeing, employee situation, and help.

Does this template apply to every employee, location, or benefit plan?

No. The example directs readers to check the audience, location, eligibility, plan year, and terms in their employer’s current approved materials. Replace Northstar-specific sample content with confirmed information for your organization. Do not treat a general hub page as a personal eligibility or coverage decision.

Who should own and maintain the benefits hub?

Assign a benefits content owner to coordinate page accuracy, corrections, and review triggers with the relevant service owners. Benefits, payroll, leave, carriers, and technical support may each own different answers; the template’s enrollment journey shows these as separate routes rather than one automatic handoff. Publish the approved help route and responsible role before launch.

How often should the pages be reviewed?

Review the relevant pages whenever plan documents, enrollment dates, provider details, eligibility rules, app instructions, or location-specific crisis resources change. Set a recurring review cadence that fits your organization’s governance, but do not imply that the sample content has already been reviewed. The contacts directory and any time-sensitive offers need particular attention when their source information changes.

Does the template provide legal or regulatory advice about benefits?

No. It is an orientation and reference site, not a substitute for plan documents, legal guidance, or official enrollment instructions. The sample explicitly tells readers to confirm current terms and applicable rules with approved sources and responsible owners. Have qualified internal reviewers validate legal, plan, privacy, and location-specific content before publication.

Can employees use the plan tabs and sample figures to choose coverage?

No. The plan descriptions, statistics, match details, dates, contacts, and other Northstar examples are illustrative and do not establish real plan options or terms. The comparison page provides questions to verify in matched, current official materials; its tabs are not a personalized recommendation. Replace or remove every sample that could be mistaken for an actual offer.

What should we customize before publishing?

Replace fictional company and app references, sample benefit details, plan-year examples, figures, contact routes, and location-specific guidance with approved information. Confirm which employee groups and locations can access each destination, and remove any section that does not fit your benefits program. Add the real content owner, correction route, review trigger, and any confirmed links through the site setup.

Does the template connect to enrollment, payroll, or benefits systems?

The pages describe destinations such as Benefits Management, Compensation Management, Company Store, and the Benefits & Enrollment Hub, but the example does not configure live integrations or submit transactions. Confirm the actual apps, access arrangements, features, and destinations in your tenant before publishing. A connected callout or sample instruction is not evidence that a system connection is active.

Why use this hub instead of a single benefits document or ad-hoc messages?

The hub gives common employee tasks distinct destinations, including comparing source documents, preparing for enrollment, checking life-event instructions, and finding the right contact. Its navigation and cross-links reduce the need to search through scattered messages, while the source-check examples help prevent mismatched plan years and assumptions. Keep authoritative plan documents in their approved home and use this site to orient readers to them.
