Frequently asked questions

Why "no names, no blame"?

Because the purpose is to prevent a repeat, and naming individuals suppresses future reporting. Describe the condition that allowed it, not the person who encountered it.

Why does this template ask for a photo?

37% of real incident posts were flagged as needing a visual. A photo of the hazard or the correct setup communicates a physical condition faster and less ambiguously than any paragraph.

Is a safety post really worth a Must-Read?

This shape had the worst risk-to-reach ratio in the entire corpus: 80% high-criticality and a 13.0% read rate. If any post type justifies acknowledgment and escalation, it is this one.
