Steps

  • Recognize arrest, call for help

Pulselessness or unresponsiveness with abnormal breathing → CALL CODE BLUE (push button or page overhead). Note time of recognition.

  • Begin high-quality CPR

Compressions 100-120/min, depth 2-2.4”, full chest recoil, minimal interruption. Switch every 2 min.

  • Code team arrives, role assignment

Team leader (intensivist/hospitalist), compressor, airway, IV/meds, recorder, runner. Assign roles aloud.

  • ACLS algorithms

Rhythm check every 2 min. Defibrillate VF/pVT. Epinephrine 1mg every 3-5 min. Identify and treat reversible causes (Hs and Ts).

  • ROSC or termination

If ROSC: post-arrest care (cooling, neuro check, transfer to ICU). If TOR: family notification per protocol.

  • Debrief

Brief debrief immediately after event. Hot wash: what went well, what to improve. Formal debrief within 7 days.
