Steps
-
Ensure scene is safe
Don’t become the next victim. Check for ongoing hazards (electrical, chemical, falling objects). Lock out / isolate before approaching.
-
Provide first aid
Trained first-aider provides care within scope (bleeding control, AED, splinting). Keep the injured person calm and conscious if possible.
-
Decide: 911 or self-transport
Call 911 for: unconsciousness, severe bleeding, breathing difficulty, possible head/neck/back injury, electrical/chemical exposure. Otherwise transport to occ-med clinic.
-
Preserve the scene
Photograph the scene before anything is moved. Mark off the area. Don’t clean up until safety has documented. Save involved equipment for inspection.
-
Identify witnesses
Get names and contact info of anyone who saw the incident. Capture initial statements while memory is fresh. Don’t coach or lead witness accounts.
-
File incident report
Complete internal incident report within 24 hours. Include facts, photos, witness names, root cause analysis.
-
OSHA 300/301 evaluation
If injury required medical treatment beyond first aid, days away from work, restricted duty, or loss of consciousness, log on OSHA 300. File 301 within 7 days.
